Not sure if Iron Dylan x or cows R us was first to 99 dungeoneering. So far, I know of StuckInADaze, 99 runecrafting. I think it would be an interesting idea to note who first reached 99 in each skill in HC, as it does broadcast it to all servers. The second safety net is to play long enough to earn an extra life. This will cost GP so players will face a tactical choice in whether they dedicate precious money towards this safety net. This is an unlock rather than a physical item (like the scroll of life from Dungeoneering).
